Job Description
Job Responsibilities:
Conduct home visits to provide assessment and services
Coordinate and develop home care plans for the patients
Provide direct nursing, psychological and emotional care to the patients
Educate caregiver and patients when required
Provide accurate evaluation and reports

Requirements:
Minimum Diploma in Nursing
SNB Fully Registered
Applicants with min 2-3 years experience in geriatric/palliative/ICU setting is a plus
