Job Description:
Job Responsibilities:
Assess the psycho-social status of the patients and families / caregivers to provide appropriate care plans
Counsel and support the patients and families / caregivers associated with the illness
Conduct home visits / care and coordinate in home modification for patients
Work closely with other disciplinary teams on patient care plans
Work with other provider to provide appropriate community services to ensure continuity of care
Share the sessions with students and other professionals
Maintain documentation for case management and audit purposes
Job Requirements:
At least Diploma / Degree in Social Work
Registered with Singapore Association of Social Work
