Roles & Responsibilities
RESPONSIBILITIES
Identify and develop training roadmaps for the PCN Care Teams (which includes Nurses, Primary Care Co-ordinators, Clinic Assistants, GPs)
Anchor the development of standardised care delivery
Involved in workgroups to support primary care development and transformation.
Co-create new models of care to support team-based care approach.
Gather insights for development and implementation of new initiatives and support the administration of the existing primary care model
Provide clinical supervision and implement framework to ensure patient safety and staff safety for care delivered
Consolidate and share best clinical practices across the primary care landscape
JOB REQUIREMENTS:
Bachelor degree Nursing and / or Master degree in any discipline
Prior experience in healthcare management or operations in primary care or hospital is desirable
Possess a high level of adaptability and learning agility in a dynamic environment
