Roles & Responsibilities
Job Descriptions:
work closely with our home care team and patients’ families to conduct home visits and care assessment for patients;
perform procedural-based nursing care, e.g. administering home antibiotic, TPN, Stoma and Wound Care, Palliative home care, etc.;
prepare patients for pre-consultation activity like drawing of blood and sending blood for laboratory analysis, etc.;
trace investigation results and follow up with doctors’ instructions;
dispense medication to patients and advise them accordingly to doctor’s instructions;
update home care team on patients’ conditions and follow up;
maintain timely and accurate documentation of patients’ condition and care given into our system for quick and accurate follow up;
maintain a safe environment and adhere to infection control practices;
participate in any other duties / projects assigned by doctor / nurse manager.
Requirements:
Valid Practising Certificate;
Valid Class 2 / 3 driving licence would be an advantage;
At least 2 years of relevant experience in community / home care visits / palliative care would be advantageous;
Professional Certificate/NITEC, Diploma/Advanced/Higher/Graduate Diploma, Bachelor’s Degree/Post Graduate Diploma/Professional Degree in Nursing or equivalent;
Valid BCLS & AED certification.
