Job Description:
Responsibilities:
Assess the psycho-social status of the patients and families / caregivers to determine the appropriate care plans
Counsel and support the patients and families / caregivers associated with the illness
Maintain working relationship with other disciplinary teams on patient care plans
Work with other provider to provide appropriate community services to ensure continuity of care
Share the sessions with students and other professionals
Maintain the right documentation for case management and audit purposes
Requirements:
Candidate must possess at Degree in Social Work
Registered with Singapore Association of Social Work
Good interpersonal and communication skills
