Responsibilities:
• Perform home visit for patients
• Assess patient’s health conditions and perform assessment
• Provide nursing care and supports to patients
• Plan and implement patient care
• Provide advise, counselling and education to patients

Requirements:
• Possesses at least a Diploma in Nursing
