Job Description:
Responsibilities:
• Provide administrative and operational support in setting up the Year of Care Program
• Develop workflow for transiting patients from the Specialist Clinics to the community
• Perform patient selection, patient counselling, appointment consolidation and referrals to primary care partners and community health resources
Requirements:
• Diploma in any equivalent fields
• Good customer service skills. Prior experience in hospitality or service industries will be an advantage.
• Good coordinating and organizing skills
• Able to work in fast pace environment
