Roles & Responsibilities
Working Hours: 5.5 days office hours
Job Scope:
Trained in chronic disease management in primary care setting
Work closely with doctors, advanced practice nurse, allied health members to manage chronic conditions
Obtain a thorough health history and assess your level of disease control
Identify factors that contribute to your sub-optimal disease control
Provide education on lifestyle modification and teach self-management to empower you to manage your health
Monitor your condition and work with your doctor to review your plan of care regularly.
Job Requirements:
Min. Diploma/ Degree in Nursing
Possess Advanced diploma/ specialist diploma in either chronic disease/ mental health/ endocrine/ diabetes/ gerontology or any other relevant field
Valid SNB & BCLS
