Roles & Responsibilities
· Evaluates, creates, implements, and monitors individualized care plans for residents with complex medical needs, following current nursing protocols and standards.
· Educates and supports residents as well as their families or caregivers to promote understanding and involvement in care.
· Leads and coordinates discharge planning efforts for residents who are deemed suitable for discharge.
· Ensures that nursing services are provided in line with recognized clinical standards and evidence-based practices.
· Keeps nursing policies and standard operating procedures (SOPs) current to ensure consistent, high-quality care delivery.
Requirement :
· More than 3 years of relevant working experience
· SNB Registered
