We are seeking a compassionate and responsible Contract Home Care Nurse to provide 12-hour private home-based nursing support for patients in the community. This role focuses on holistic care, daily assistance, and close companionship to support patients’ well-being and independence at home.
Key Responsibilities:
-Provide 12 hours of private home-based nursing care per shift
-Assist patients with Activities of Daily Living (ADLs) and personal care
-Support patients in their daily routines, including mobility and engagement
-Accompany patients to clinic appointments, physiotherapy sessions, or medical visits when required
-Observe patients’ condition and provide daily nursing reports and updates
-Deliver care with empathy, professionalism, and respect for patient dignity
Work closely with the care coordination team to ensure continuity of care
Requirements:
– SNB-registered nurse (Registered Nurse or Enrolled Nurse)
-Singapore Citizen or Singapore Permanent Resident (SPR)
-Bilingual will be an advantage
-Possess a serving heart, patience, and a strong, calm personality
-Able to work independently in a home-care environmentPrior home care or community nursing experience is an advantage
