Job Description:
Responsibilities:
• Assess the psycho-social status of the patients and families / caregivers to determine the appropriate care plans
• Counsel and support the patients and families / caregivers associated with the illness
• Conduct home visits / care. When necessary, coordinate home modification for the patients
• Maintain working relationship with other disciplinary teams on patient care plans
• Work with other provider to provide appropriate community services to ensure continuity of care
• Share the sessions with students and other professionals
• Maintain the right documentation for case management and audit purposes Candidate must possess:
• At least Diploma / Degree in Social Work
• Registered with Singapore Association of Social Work
• At least 2 years of related working experience
